The Assertive Community Treatment Team is a multi-disciplinary team that provides intensive case management focusing on support, rehabilitation and treatment to individuals with severe and persistent mental health issues. The program helps consumers achieve their goals, improve their health and quality of life, and live as independently as possible.
Team members’ specialties include psychiatry, registered nursing, social work, occupational therapy, addictions, therapeutic recreation and peer support.
Referral Process – An ACTT Referral Form for Waterloo Region needs to be completed. The form is available on this website by clicking here. The form is also available from the Grand River Hospital Crisis Clinic, COTT (Community Outreach Treatment Team), Regional Mental Health Care – London, Canadian Mental Health Association – Grand River Branch, Waterloo Wellington Community Care Access Centre, family physicians, and psychiatrists.
New referral applications are reviewed by a Community Intake Group, which is comprised of Cambridge Memorial Hospital, Canadian Mental Health Association – Grand River Branch, Waterloo Wellington Community Care Access Centre, Grand River Hospital, St. Joseph’s Health Care – London, Waterloo Region ACT and Transition Teams, and WRHMH. Each referral is assessed to determine if ACTT is the appropriate level of support required.
Applicants may be placed on a wait list.
