Long Term Support Services

All individuals housed by WRHMH receive a range of ongoing support services that respond to their individual needs. 

Community support coordinators, practical support workers, and nurses, in partnership with our consumers, assist them in recovery by working on identified goals.  The focus is on strengths and includes goals such as improving financial planning and physical health, as well as expanding social networks and participation in community activities. 

An indication of health and well-being for individuals is a reduction in hospitalizations.  Over the past 10 years, there has been a decline in the number of our consumers requiring hospitalization.  Although many factors in an individual’s life may contribute to this decline over time, individuals in housing and long term support experience a significant decrease in hospitalizations.


Referral Process –for Housing, Outreach and Short Term Support Services, and Long Term Support Services.  Assistance is available to eligible individuals with a mental health issue, age 16 years or older.  Priority will be given to individuals who are homeless or at risk of homelessness.  To apply, please contact the Community Intake Coordinator for an appointment.  Intake staff will interview you at your home or the location of your preference.  The interview usually takes one hour.  Eligible applicants may be placed on a wait list.
